Membership Cancellation Request Form

Instructions:

We're sorry you need to cancel your membership.

All memberships are for a minimum period of time and/or number of sessions and as such, cannot be cancelled without reason. The Membership Cancellation Policy and those terms are outlined in your Membership Contract and summarized below.

Memberships may only be cancelled for three reasons. They are:

• Relocation greater than 10 miles from the studio.
• Injury or Illness that prevents you from doing any type of exercise offered at Vitruvian Fitness.
• Significant change in financial status that prevents you from being able to finish your membership term.

We ask for 5 business days notice to cancel your membership.

When your membership is cancelled, any sessions or time left on the membership will be forfeited effective the last day of your membership.

Your Return

If you are able and interested in training with us again, we will welcome you back with open arms when the time and circumstances are right. 

Only our future current membership options and prices will be available to you with no guarantee that any pricing or discounts you currently enjoy will be available then.
